آلرژن های شایع در بیماران آتوپیک
Common Allergens in Patients with Atopic Dermatitis

چکیده
زمینه و هدف: مواجه با آلرژن ها و به دنبال آن تولید IgE اختصاصی در ایجاد بیماری درماتیت آتوپیک، شناسایی آلرژن های مسبب بیماری و انجام ایمونوتراپی در درمان بیماری نقش بسزایی خواهد داشت. هدف از این مطالعه تعیین فراوانی آلرژن های شایع در بیماران درماتیت آتوپیک بود.
روش بررسی: در این مطالعه توصیفی-تحلیلی، سطح سرمی IgEتوتال و فراوانی IgE اختصاصی به روش ایمونوبلاتینگ علیه 20 آلرژن شایع در 150 نفر از بیماران درماتیت آتوپیک در سال1391 -1389 مورد بررسی قرار گرفت. گروه شاهد شامل افرادی بود که سالم تشخیص داده شده بودند.
یافته ها: میانگین سنی بیماران بررسی شده 79/14±02/30 سال بود که 77 نفر (3/51%) مذکر و 73 نفر(7/48%) مونث بودند. در 90 درصد بیماران سطح سرمی Total IgE بیش تر از حدوده مرجع با میانگین سطح سرمی Total IgE IU/ml 103 ±51/227 بود. بیش ترین فراوانی آلرژن به ترتیب مربوط به گرده گندم زار (6/48%)، چمن دم گربه ای (9/42%)، مایت خانگی (7/22%)،گربه (7/16%)،اسب (10%)، درختان خیابانی (33/11%)، سیب زمینی(33/11%)، سگ(7/16%)، سفیده تخم مرغ(7/8%)، شیرگاو(7/8%) بود. فراوانی آلرژن های مثبت در گروه گیاهی و قارچ 34/54 درصد، در گروه حیوانی 08/26 درصد و در گروه غذایی 56/19 درصد بودند. در گروه شاهد افزایش Total IE یافت نشد.
نتیجه گیری :براساس یافته های حاصل از این مطالعه آلرژن های شایع در آذربایجان شرقی مشخص گردیدکه با علم بر این ها می توان برای بیماران آلرژیک آتوپی پرهیز از برخورد با آنها را توصیه و آموزش لازم را داده و امکانات ایمونوتراپی را نیز براساس آلرژی اختصاصی بیمار فراهم نمود.

Abstract
Background and objective: Being exposed to different allergens, followed by the production of specific IgE, has an important role in causing atopic dermatitis, recognizing the allergens and applying immunotherapy for treatment. We aimed to determine the frequency of common allergens in the patients suffering from atopic dermatitis.
Material and Methods: In this descriptive- analytical study the serum level of total IgE and frequency of specific IgE were measured by Immunoblotting method against 20 common allergens in 150 atopic dermatitis patients in 2010-2011 .The control group included the individuals who have been diagnosed healthy.
Result: The mean age was 30.02±14.79 the participants were male (77 51.3%) and female (73 48.7%). In 90% of patients, total IgE was more than the reference range with the mean of 227.51±103IU/ml. The most frequent allergens related to: Cultivated rye (48.6%), Timothy grass (42.9%), House dust mite (22.7%), Cat (16.7%), Horse (10%), Birch (11.33%), Potato (11.33%), Dog (16.7%), Egg white (8.7%), Cow milk (8.7%). The frequency of positive allergens in plants and fungus group was 54.34%, in animals was 26.8% and in foods was 19.56%. In control group, there was no total IgE elevation.
Conclusion: Based on the recognition of common allergens in east Azerbayjan, Iran, we recommend avoiding of these allergens and providing immunotherapy.
