فراوانی هپاتیت C, B و عفونت با HIV در اهدا کنندگان خون و بیماران مراجعه کننده به سازمان انتقال خون گرگان در سال 1382

زمینه و هدف: مشاوره قبل از اهدا خون و انجام آزمایش ها غربالگری از نظر بیماریهای منتقله شایع از طریق انتقال خون نظیر هپاتیت B و C و ایدز در تضمین سلامت خون از اهمیت به سزایی برخوردار است. این مطالعه به منظور تعیین فراوانی این بیماری ها نزد اهدا کنندگان خون با مراجعین سرپایی به آزمایشگاه انتقال خون و بررسی صحت روش های تشخیصی الیزا انجام گردید.روش بررسی: این مطالعه توصیفی روش آزمایش های HBSAg، HCV Ab و HIV Ab به روش الیزا روی کلیه نمونه خون های اهدا شده در پایگاه انتقال خون گرگان و بیماران مراجعه کننده به آزمایشگاه این مرکز در سال 1382 انجام گرفت و در صورت مثبت بودن آزمایش الیزا آزمایش های تاییدی نوترالیزاسیون برای HBSAg، RIBA برای HCV Ab و Western blot برای HIV Ab انجام گردید.یافته ها: 5.2 درصد اهدا کنندگان در مقایسه با 13 درصد بیماران از نظر HBS Ag و 3 درصد اهدا کنندگان در مقایسه با 8.3 درصد بیماران از نظر HCV ab و 35.0 درصد اهدا کنندگان در مقایسه با 04.2 درصد بیماران از نظر HIV مثبت بودند. همچنین در اهدا کنندگان 92 درصد موارد واکنش دار از نظر HBS Ag، 20.1 درصد از نظر HCV Ab و صفر درصد از نظر HIV به روشهای الیزا در روش های تاییدی نیز مثبت شدند. در حالی که در بیماران مراجعه کننده 97 درصد موارد واکنش دار از نظر HBS ag و 47 درصد از نظر HCV ab و 60 درصد از نظر HIV به روش الیزا در روشهای تاییدی مثبت شدند.نتیجه گیری: این مطالعه نشان داد میزان فراوانی هپاتیت C، B و ایدز نزد بیماران سرپایی که به نحوی جز گروه های پر خطر محسوب می شوند، بیشتر از اهدا کنندگان بوده است و نیز آزمایش HBSAg در مقایسه با آزمون های HIV و HCV به روش الیزا از صحت بالاتری برخوردار است.
Background&Objective: Consultation prior to blood donation and screening tests of common blood born diseases such as Hepatitis B,C and AIDS are very important issue in blood quality assurance, therefore we decided to compare frequency of such diseases in blood donors and outpatients admitted in the blood transfusion centers. Materials&Methods: For this purpose the initial tests including HBsAg, HCVAb, and HIVAb were performed on all blood donors and outpatients' blood samples in Gorgan transfusion center by ELIZA methods during 2003 and confirmatory tests including neutralization for HBsAg, RIBA for HCVAb and Western blot for HIVAb performed in condition where ELIZA report was positive. Results: The blood donor in this study had the following positive results: HbsAg 2.5%, HCVAb 3%, HIV 0.35%. The same indeces for the out patients were positive as follow: 13%, 8.3% and 2.04% respectively. The other finding from this investigation indicate that the blood donors positive cases with the ELISA technique were confirmed by Western blot 92%, 20.1% nad 0% for HBSAg, HCVAb, as HIV respectively. The confimatory results of the positive out patient's cases were as follow, 97%, 47% and 60% for HBSAg, HCVAb and HIV respectivley. Conclusion: This study reveals frequency of Hepatitis B and C and AIDS among outpatients which considered being high risk group are more than the blood donors. We conclude that the measurement of HBSAg by ELIAZ methodes in comparision to the HCV and HIV are more accurate.
