Abstract

Background and objective:

Delirium is an acute and transient disorder in the function of the brain. Although the main core of this syndrome is consciousness disorder and deficiencies in attention and concentration, the general deficiency is seen in all psychological areas of thinking, temperament, cognition, language, speaking, sleeping, and mental-motional and other cognitive areas. Delirium is often seen in recovery room and is a predictor of post-operative delirium in the general ward. This study was conducted to determine the incidence of delirium in patients after surgery in the recovery room.

Materials and Methods:

This descriptive cross-sectional study was conducted on 75 patients after general and orthopedic surgery and in the recovery room of the educational-therapeutic center of Gorgan in 1389 using the DESC-Nu nursing delirium screening scale. Data was analyzed using chi-square descriptive and analytical statistics and T-test.

Results:

Among the 75 studied patients in the recovery room after orthopedic surgery and general surgery, 53.3% were women with a mean age of 48.7. The mean surgery duration was 129.21 minutes. 26.6% had been under spinal anesthesia and 73.3% had been under general anesthesia. Delirium was observed in 30.6% of all the patients. Delirium was observed in 21.3% of patients having orthopedic surgery and 9.3% of the patients having general surgery. Delirium had a significant statistical relation with the variables of age, gender, and type of surgery (p<0.05).

Conclusion:

This study showed that 30.6% of patients had delirium. Male and older patients having orthopedic surgery were more vulnerable. Therefore, usual assessment of delirium in recovery room to identify patients with delirium can be a guide of nurses’ appropriate care of patients after surgery.

Abstract

Background and Objectives: With respect to lack of information about the effect of intensity differences, type of sport participation and their relationships with the other health factors such as cardiorespiratory fitness (CRF), the present study was conducted to evaluate the associations between sports participation, levels of Physical Activity (PA) and CRF.

Methods: The present analytical study was conducted on 406 undergraduate students aging 18-21 years from Gorgan University of Agricultural Science and Natural Resources in second semester of 2014. CRF was tested with Rockport Walk Test among all participants. Sports participation was assessed by Ledent questionnaire and habitual physical activity (PA) data was evaluated using metabolic equivalent (METs) self-reported questionnaire. All analyses were carried out using SPSS version 20.0.  An independent T-test, Chi-square statistic, ANOVA and regression were employed to analyze data statistically.

Results: The participants with desired CRF and acceptable levels of PA were significantly engaged more active in organized or unorganized sport activities and also competitive sports compared to others (P < 0.05). Individuals with at least 3 hours of moderate to vigorous PA per day had a better CRF (P<0.05).Regression results showed that participation in any level of competitive sports regardless of winning or loss guarantee the CRF to some extent.

Conclusion: Participation in competitive sports along with adopting active life style (2:30 min PA with moderate intensity and 30 min of competitive sport) may increase the chance of gaining CRF.

Abstract

Background & Objective: Inability to measure exact exposure in epidemiological studies is a common problem in many studies, especially cross-sectional studies. Depending on the extent of misclassification, results may be affected. Existing methods for solving this problem require a lot of time and money and it is not practical for some of the exposures. Recently, new methods have been proposed in 1:1 matched case–control studies that have solved these problems to some extent. In the present study we have aimed to extend the existing Bayesian method to adjust for misclassification in matched case–control Studies with 1:2 matching.

Methods: Here, the standard Dirichlet prior distribution for a multinomial model was extended to allow the data of exposure–disease (OR) parameter to be imported into the model excluding other parameters. Information that exist in literature about association between exposure and disease were used as prior information about OR. In order to correct the misclassification Sensitivity Analysis was accomplished and the results were obtained under three Bayesian Methods.

Results: The results of naïve Bayesian model were similar to the classic model. The second Bayesian model by employing prior information about the OR, was heavily affected by these information.

The third proposed model provides maximum bias adjustment for the risk of heavy metals, smoking and drug abuse. This model showed that heavy metals are not an important risk factor although raw model (logistic regression Classic) detected this exposure as an influencing factor on the incidence of lung cancer. Sensitivity analysis showed that third model is robust regarding to different levels of Sensitivity and Specificity.

Conclusion: The present study showed that although in most of exposures the results of the second and third model were similar but the proposed model would be able to correct the misclassification to some extent.

Abstract

The dialkyl- or alkyl/aryl esters of 1, 2-benzenedicarboxylic acid, which are known as Phthalates, are high-production volume synthetic chemicals and considered as environmental pollutants, due to high production and uses in community, plastics industry and common consuming products. Di-(2-ethylhexyl) phthalate (DEHP) is the most abundant phthalate in the environment. Human exposure with DEHP could be done via different chemical compounds including food packaging, household furnishings, nutritional supplements, cleaning materials and insecticides. Besides, exposure of human with phthalates occurs through different pathways such as direct contact and using Phthalate-containing products, and indirectly through leaching into other products, or general environmental contaminations. Historically, the diet has been considered the major source of phthalate exposure in the general population, but in all sources, pathways, and their relative contributions to human exposures are not well understood. Medical devices are other source of significant exposure in human. Furthermore, cosmetics, personal care products, pharmaceuticals, nutritional supplements, herbal remedies and insecticides, may result in significant but poorly quantified human exposure with this compounds. In the present review article, we tried to discuss about metabolism of phthalates in human, toxicity, monitoring of phthalates in foods, environment, and cosmetic products and then metabolites of phthalates. Finally, evaluation of human exposure through biological control is discussed.

Abstract

Background and objectives: Admission to university is an important event in the life of youth in every country. Homesickness is one of the most significant issues faced by university students during their education life. The present study aimed to determine the association between homesickness, happiness and the related factors in the dormitory students in Hamadan University of Medical Sciences, Iran.
Methods: This cross-sectional, descriptive-analytical study was conducted on 387 dormitory students at Hamedan University of Medical Sciences in Hamedan, Iran during February-March 2015. Subjects were selected via stratified random sampling with proportional allocation. Data were collected using self-report and using demographic and contextual questionnaires, Fenfelit homesickness questionnaire, and oxford happiness questionnaire. Data analysis was performed in SPSS version 23 using Pearson’s correlation-coefficient and general linear model (GLM) at the significance level of 0.05.
Results: The mean score of homesickness in both genders had a significant difference (P=0.009). In addition, the mean score of happiness differed in terms of economic status. Homesickness was negatively correlated with the happiness score, so that increased homesickness was associated with reduced happiness. Variables of homesickness, age, internet use, maternal education, health status, economic status, and education level could significantly predict happiness.
Conclusion: Mental health of students in dormitories is of paramount importance. Therefore, interventions and preventive programs must recognize the causes of happiness, and appropriate interventions should be developed and implemented based on the influential factors in this regard.

Abstract

Background and objectives: It has been reported that deep-fried oils (DFOs) used in the preparation of fast foods can affect the immune system. On the other hand, regular physical activity and proper nutrition, in addition to affecting physical function, have a tremendous impact on the immune system. The aim of the present study was to investigate the anti-inflammatory effects of aerobic training (T) and octopamine (O) consumption in the heart tissue of rats fed DFO.
Methods: In this experimental study, 25 male Wistar rats with mean age of 20 weeks old and weighing 300- 350 were purchased and divided into 5 groups of 5 rats, including: 1) control, 2) DFO, 3) DFO+T, 4) DFO+O and 5) DFO+T+O. During four weeks, groups 2- 5 received DFO by gavage; groups 3- 5 ran on treadmill (with speed of 50% Vo2max in the first week which reached 65% Vo2max in the last week) five sessions per week and 20 minutes per session and groups 4-5 received 81 μmol/kg octopamine supplement peritoneally for 5 days per week. Nuclear factor kappa B (NF-κB) and tumor necrosis factor-alpha (TNF-α) gene expression were measured in heart tissue.
Results: DFO significantly increased NF-κB (P=0.005) and TNF-α (P=0.001) gene expression levels; T significantly on decreased NF-κB (P=0.01) and TNF-α (P=0.007) gene expression levels; O consumption significantly decreased NF-κB (P=0.001) and TNF-α (P=0.001) gene expression levels; however, the interactive effects of T and O consumption on NF-κB (P=0.57) and TNF-α (P=0.20) gene expression levels was not significant.
Conclusion: It seems that T and O consumption alone have anti-inflammatory effects on the heart tissue of rats fed DFO, however T and O do not have anti-inflammatory interactive effects.

Abstract

Background and objectives: Controlling nutrition and exercise can be two important strategies in controlling tendon health. It has been reported that resistance training and palm pollen separately can improve Scleraxis (Scx) in tendon tissue; so present study aimed to investigate the interactive effects of resistance training with ethanolic extract of palm pollen on Scx protein and gene expression levels in the tendon tissue of male adult rats.
Methods: In this experimental study 30 male adult rats divided into 6 groups of 6 rats including: 1) sham, 2) training, 3) palm pollen, 4) testosterone, 5) training + palm pollen, and 6) training + testosterone. During 4 weeks, groups 2, 5, and 6 performed resistance trainings for five sessions per week; groups 3 and 5 received 100 mg/kg palm pollen for five days per week via gavage and groups 4 and 6 received 2 mg/kg testosterone propionate peritoneally. Scx protein and gene expression levels were measured in tendon tissue by Western blot and real-time PCR methods respectively. Shapiro- Wilk, one way ANOVA with Tukey’s post- hoc tests were used to analyze the findings (P≤0.05).
Results: Training significantly increased Scx protein levels (P=0.005); palm pollen significantly increased Scx gene expression levels (P=0.001); training + palm pollen significantly increased Scx protein and gene expression levels (P=0.001) also training + palm pollen had more favorable effect on increase of Scx protein and gene expression levels compared to training and palm pollen alone (P=0.001).
Conclusion: It seems that resistance training simultaneously with palm pollen administration can have a more favorable effect than each one alone on improving Scx protein and gene expression levels in tendon tissue of male adult rats.

Abstract

Background and objective: Exercise and nutrition are two factors influencing the improvement of inflammatory markers in patients with colon cancer. Aim of present study was to investigate the effect of aerobic training (AT) with Purslane (Portulaca Oleracea) Seed (PS) on toll like receptor 2 (TLR-2) and TLR-4 in colon tumor tissue of rats with colon cancer.
Methods: In this experimental study 30 adults rats were divided into five groups of six rats including: 1) healthy control, 2) control, 3) training, 4) PS, and 5) training + PS. Colon cancer induced by intra-peritoneal injection of azoxymethane in groups 2- 5. During eight weeks, groups 3 and 5 performed AT for five sessions per week also groups 4 and 5 received 75 mg/kg PS intra-peritoneally. TLR2 and TLR4 protein levels were measured by ELISA method. For review the normal distribution and data Shapiro- wilk was used and for statistical analysis of data one way ANOVA with Tukey’s post- hoc tests were used (P≤0.05).
Results: Training had not significant effect on TLR-2 (P=0.91) and TLR-4 (P=0.95); PS and training + PS significantly decreased TLR-2 and TLR-4 (P=0.001) also training + PS had more favorable effect on decrease of TLR-2 compare to training and PS alone (P=0.001).
Conclusion: Although PS alone can improve TLR-2 and TLR-4 levels in colon tumor tissue of adult rats with colon cancer, nevertheless it appears that AT along with PS have more favorable effects on improvement of TLR-2 compare to training and PS alone.

Abstract

Background and objectives: Proper nutrition and exercise are two effective factors in improving liver function in old age. The aim of this study was to investigate the effect of aerobic training (T) with consumption of Citrus aurantium (CA) on SIR1 and PGC-1α gene expression levels in the liver tissue of elderly rats.
Method: In this experimental study, 25 elderly female rats were placed in five groups of 5 rats, including 1) control, 2) sham, 3) CA, 4) T and 5) T+CA. Over the course of eight weeks, groups 4 and 5 ran on the treadmill three sessions per week at 65 to 75 percent of the maximum running speed, and groups 3 and 5 received 300 mg/kg/day of CA extract peritoneally. Forty- eight hours after the last training session and CA consumption, SIR1 and PGC-1α gene expression levels in the liver tissue were measured by real-time PCR method. In order to analyze the findings, the Shapiro-Wilk, one-way ANOVA with Tukey’s post- hoc tests were used (P≤0.05).
Results: T had a significant effect on increasing SIR1 (P = 0.009) and PGC-1α (P = 0.001) gene expression levels; CA had a significant effect on reducing SIR1 gene expression levels and increasing PGC-1α (P = 0.001); T + CA had a greater effect on increasing PGC-1α gene expression levels than T and CA (P = 0.001).
Conclusion: Although eight weeks of T and CA consumption alone appear to improve PGC-1α gene expression levels in the liver tissue of elderly rats, concurrent T and CA consumption has more favorable effects than each of them alone.

Abstract

Background and Objective: One of the important and common applications of telemedicine is the diagnosis and treatment of skin diseases. However, teledermatology requires further commitments to provide standard services and appropriate electronic data transfer for reliable diagnostics to comply with technical standards and legal requirements. The present study aimed to determine the factors affecting the establishment of Teledermatology in the qualitative and quantitative sections .
Material and Methods: A mixed-method study (including quantitative and qualitative sections) was conducted in 2018. In the qualitative section, experts were selected by purposive sampling and the data were collected through 2 focus group discussions and 6 semi-structured interviews. The content of sessions and interviews were transcribed and then analyzed by the content analysis method. In the quantitative section, 384 participants were selected randomly. The data were collected using a researcher-made questionnaire whose validity and reliability were confirmed. Data analyses were performed in SPSS.
Results: A total of 8 categories and 58 subcategories were extracted. The 8 categories of the factors affecting the establishment of teledermatology in Iran included service provider, service recipient, structural, technological, economic, policymaking, legal, and cultural. Cultural factors (3.95±0.77) and policymaking factors (3.22±0.84), respectively, had the highest and the lowest effects on the establishment of teledermatology in Iran.
Conclusion: Factors affecting the establishment of Teledermatology are diverse and extensive, which the most effective factors were cultural factors, technological factors and factors related to service providers. These findings can help the policymakers and managers interested in the establishment of teledermatology in Iran.

Abstract

Background and Objective: Nutrition and physical activity are two factors which affecting the control of body composition and cardiorespiratory fitness in the elderly population. Present study aimed to investigate the effects of continued training (CT) and high intensity interval training (HIIT) along with Citrus aurantium (Ca) on aerobic power and body composition of elderly rats.
Material And Methods: In this experimental study, 42 elderly rats with mean age of 14- 18 months were divided into 7 groups of 6 rats including: control, sham, HIIT, CT, HIIT+Ca and CT+Ca groups. During eight weeks, the Ca groups received 300 mg/kg Ca peritoneally and CT groups ran on treadmill for five sessions per week with intensity of 85%- 100% of VO2max and speed of 15-25 m/min as well as HIIT groups ran on treadmill for five sessions per week with intensity of 65% of VO2max and speed of 20-25 m/min.
Results: CT (P=0.04), HIIT+Ca (P=0.04), and CT+Ca (P=0.04) significantly increased aerobic power; HIIT+Ca (P=0.02) and CT+Ca (P=0.03) significantly increased heart weight and HIIT+Ca significantly decreased adipose tissue weight (P=0.01).
Conclusion: Although CT can improve aerobic power in elderly rats, nevertheless it seems that CT and HIIT along with Ca administration can have more favorable effects on the body composition of elderly rats.

Abstract

Background and Objective: Interactive effects of Aerobic Training (AT) and Royal Jelly (RJ) consumption is still not well understood in Multiple Sclerosis (MS). Therefore, the aim of this study was determine the effect of aerobic training with royal jelly consumption on health related anthropometric markers in an Experimental Autoimmune Encephalomyelitis (EAE) model.
Material and Methods: 49 EAE induced by complete Freund’s Adjuvant, model female Sprague-Dawley rats were assigned to 7 groups: (1) EAE, (2) sham (Sh), (3) RJ50 mg/kg, (4) RJ100, (5) AT, (6) AT+RJ50, (7) AT+RJ100. Also, 7 healthy rats were put in the healthy control group (HC). AT was performed for five weeks, four sessions per week at a speed of 11-15 m/min for 30 minutes and RJ was IP injection for 50 and 100 mg/kg/day.
Results: In the AT, AT+RJ50, AT+RJ100 and RJ100 groups, weight, visceral fat weight and aerobic power were higher than the EAE group (P≥0.05); in the RJ50 group, weight and in the RJ100 and AT+RJ100 groups, BMI was lower than the EAE group (P≥0.05). AT+RJ100 reduced food intake compared to the EAE group (P≥0.05); also, aerobic power in the AT, AT+RJ50 and AT+RJ100 groups was higher than the RJ50 and RJ100 groups (P≥0.05).
Conclusion: Generally AT and RJ alone improved anthropometric markers in EAE model; also interaction of AT and RJ was dependent of dosage of RJ so that the effect of 100 mg/kg dosage was higher than 50 mg/kg.

Abstract

Background and objectives: One of the potentially effective methods for relieving postpartum perineal pain is auricular acupressure. This review aimed to evaluate the clinical effect of auricular acupressure on postpartum perineal pain in a systematic review.
Material and Methods: An extensive systematic search was performed on online electronic databases, including Scopus, PubMed/MEDLINE, ProQuest, Web of Science, and Cochrane using the keywords related to the purpose, from the earliest to June 30, 2021.
Results: Of 263 articles, three studies were included in the review. A total of 386 women with a mean age of 29.79 years were included in the review. Of the participants, 192 women were in the intervention group. Of the studies included, two studies evaluated auricular acupressure, and one study evaluated battlefield auricular acupuncture plus standard analgesia. Overall, the number of acupressure points varied from 3 to 5. In most cases, auricular acupressure was used at points of Shen Men (n=3) and the external genital area (n=2). Overall, auricular acupressure in one study out of three studies had a significant effect on reducing postpartum perineal pain. Two out of three studies reported no side effects of auricular acupressure for participants.
Conclusion: It seems that auricular acupressure can be used as a simple, low-cost, and practical intervention to reduce postpartum perineal pain. However, future well-designed studies are needed to confirm the findings of this systematic review.

Abstract

A Dentigerous Cyst (DC) is the most common developmental odontogenic cyst that mostly happens in patients between 10 to 30 years of age. It is usually completely asymptomatic and is discovered on a routine radiographic examination or to evaluate the reason for an erupted tooth. Radiographically, this cyst usually presents a well-defined unilocular radiolucency attached to CEJ of an unerupted tooth which can cause displacement of adjacent tooth or resorption.
This article reports a large infected DC in a 10-year-old male patient related to first and second premolar maxillary teeth, which did not erupt or even change their positions. DC was treated successfully by decompression. After 2 years follow up, there was no evidence of recurrence. In conclusion, marsupialization of cyst with no orthodontic intervention or tooth extraction is a conservative, efficient protocol that stimulates bone healing and promotes the eruption of unerupted teeth even if they are deeply crowded.
